 (1) Mẫu này dùng cho công dân Việt Nam, người nước ngoài thường trú tại Việt Nam và người nước ngoài tạm trú tại Việt Nam từ 6 tháng trở lên có nhu cầu mời, bảo lãnh cho người nước ngoài nhập cảnh thăm thân. Mỗi người kê khai 1 bản gửi trực tiếp đến Cục Quản lý xuất nhập cảnh – Bộ Công an. 
        The Vietnamese citizens, the foreigners being a permanent resident or being granted a length of stay exceeding six months in Viet Nam use this form in making application for entry into Viet Nam of his/her sponsored relative visitors. To apply, please submit a completed form in person at the Immigration Department - Ministry of Republic Security.
 (2) Công dân Việt Nam và người nước ngoài thường trú thì ghi địa chỉ thường trú; người nước ngoài tạm trú thì ghi địa chỉ tạm trú. 
         Please state the permanent residential address in Viet Nam of a Vietnamese citizen or a foreign permanent resident; state the temporary residential address if a non-permanent resident.
 (3) - Nếu người bảo lãnh là công dân Việt Nam hoặc người nước ngoài thường trú thì phải có xác nhận của UBND phường, xã nơi người  đó thường trú. 
         In case the sponsor is a Vietnamese citizen or a foreign permanent resident in Viet Nam, certification from the People's Committee of the local Ward/Village where he/she resides is required.
      - Nếu người bảo lãnh là người nước ngoài đang tạm trú ở Việt Nam thì phải có xác nhận của cơ quan, tổ chức nơi người đó đang làm việc, học tập. 
         If the sponsor is a foreign non-permanent resident in Viet Nam, the certification from his/her local employer or receiving agency/organization is required.
